The Duke
Animal Protocol Form

The Duke IACUC requires the use of this form for all new protocols, and
3 year re-writes (de novo reviews)).

INSTRUCTIONS FOR COMPLETING THE NEW PROTOCOL FORM:

    * To complete the form and save a copy on your hard drive, you will need:
   *   Adobe Acrobat version 6, 7, or 8, (available at the Duke store) or
   *  Adobe Reader 8 (or newer).  Click here to link to Adobe for a free download.
    * The new form consist of two PARTS, each part has multiple SECTIONS. 
    * Every application for animal use MUST include all of PART I; BUT ...
    * You will ONLY complete those SECTIONS of PART II that are applicable.
    * SECTION A6 (in PART I) will help you identify which SECTIONS of PART II you will need to complete. 
  * You may submit your application PART I & applicable SECTIONS of PART II by one of the following methods:  
      * Email the completed PART I & SECTIONS to: IACUC@DUKE.EDU
        * This is the PREFERRED option, but any of these methods will work.
        * If sent from the PI's Duke email account, signature are not required.
      * Print off a hard copy of the completed PART I & SECTIONS and FAX to:  919.668.6725
      * Print off a hard copy of the completed PART I & SECTIONS and hand carry to:  Hock Plaza Building; Suite 1104.
      * Print off a hard copy of the completed PART I & SECTIONS and mail to:  Office of Animal Welfare Assurance; 2424 Erwin Road, Suite 1104; Durham, NC 27705

  PART II:  Project Specific Information.  Only complete those sections that are applicable to your study.
