Controlled Substance Forms

  • Federal DEA on-line forms (for download and ordering):  Click here to go to the DEA website to download or order DEA specific forms.

  • NC State Controlled Substance License Application (NC DHHS Form 225):  This is the state of North Carolina application form you submit to obtain a controlled substance license.  There is no on-line form for this license.  This is the FIRST step in the process of obtaining a RESEARCHER license.  Obtaining the state license number is required prior to requesting a federal license.

    • NC State Instructions for DHHS 225:  These are the instructions for completing the state application form.  You may apply as an individual or as a group (division or department level).  The instructions apply to both circumstances.

  • Federal Controlled Substance Application (DEA Form 225): This is the on-line federal application form you submit to obtain a RESEARCHER license.  This is a different license from a practitioner license for physician or veterinary clinical care use of controlled substances (which uses DEA form 224).  A practitioner license is for clinical care' a researcher license is for experimental use.

  • Transfer of Schedule II Substances (DEA Form 222):  These forms are uniquely numbered by the DEA. You cannot use a photocopy or web copy of this form.  DLAR keeps some of these forms on file and may be able to help you with completing this form.  Registrants can also order these forms directly from DEA.  If you wish to order these forms, please visit the DEA web site by clicking this link.  You will need to have a license for Schedule I or Schedule II controlled substances to order these forms from DEA.

  • Annual Report of Controlled Substance Use (DEA Form 227):  This form is sent from the DEA directly to the registrant approximately 60 days before expiration of the license.  While there is no annual report to Duke, registrants should assure that the OESO database is accurate (changes in controlled substances, changes in location, etc.).  An annual update of the OESO database is a good idea.

  • Duke Forms:  Suggested Forms for Lab Management of Controlled Substances:  The forms below are suggested templates for managing controlled substances in your laboratory.  These forms are not required - any hard copy record (not computer record) that keeps the required information is acceptable.  See the Duke Controlled Substance Policy for details of what maintained information is required. The first three forms are PDF pages which you will keep in any laboratory notebook. The last for is an Excel spreadsheet that you can download and modify to suit your specific laboratory needs (as long as it meets the policy requirements):
